As a BCSA candidate, your signed application form is vital to confirm the numbers and information reported in your online application form.
We cannot process and send results from your practical assessment before your signed application form is uploaded to your account.
When should I upload this?
The signed application form should be uploaded prior to your practical exam. This is to ensure there is no delay in processing your results. Centres may not allow you to take the practical exam at their centre if you have not yet uploaded your signed form to your account. You will not need to sent this to them directly once you've uploaded this; they will have access to your signed form as soon as you upload to your BCSA account.
How do I upload this?
You can upload your signed BCSA application form to your BCSA account as soon as your online form has been approved. This is actioned from the 'application' tab in your account:
You can then download the pdf of your approved online application, which needs to be signed and dated by the screening colonoscopy site endoscopy lead, and the screening centre director, as well as yourself.
We can only accept the form if this is all submitted as one document (ie not just the signatures, nor the pages submitted individually). This is to confirm that your data has been reviewed by the above individuals and confirmed.
Once you have secured all signatures, you must upload the completed form to your account, as seen in the first screenshot:
Successful candidates are not considered certified until the form has been returned and the BCSA certificate received.
