Do I need to submit any further documentation with the new screener request form?

For bowel scope, you do not need to submit any further information. The application will go to your regional QA advisor for review and they will request more information if it is needed. 

For colonoscopy, when you submit the new screener request form, you will also need to provide:

• an OBIEE report of colonoscopists and their procedures for the last year individually broken down.
• a 12 month capacity plan. 

For both pathways, please ensure you include the number of BCSP lists undertaken by current accredited endoscopists and the number of BCSP lists for the proposed candidate.
